PCN nurse discussion with patients

Across the Yorkshire Coast and Wolds Primary Care Network, we are working with local partners to take health and wellbeing support beyond healthcare settings and into the heart of our communities.

Our projects reflect the needs of the people we serve. Blossom uses creative, art-based activities with Driffield School to support young people’s emotional wellbeing and connection. Our community outreach work with Love Driffield and local food banks offers advice, blood pressure checks and help accessing support. In our rural areas, the Farming Health and Wellbeing Project brings a range of services into farming communities, helping reduce barriers created by long hours, isolation and distance.

Together, these projects demonstrate how partnership working can make support more accessible, local and responsive.
