Practices working together in a primary care network are able to decide how they can transform patient care for the better by sharing resources and working at scale and in cooperation with each other. 

Whilst individual practices continue to serve their own patients, when they come together with other local practices, they are able to use additional funding more effectively and with greater efficiency.

As our network develops, practices will carefully assess data and listen to their patients in order to understand the priorities in our area. This process has already started and we are developing further initiatives
