Taking Health and Wellbeing into the Community
Working with Love Driffield and the local Food Banks to make health support more accessible.

Sometimes, making a difference starts with a simple conversation.

Our PCN Social Prescribers and Care Coordinator have been taking health and wellbeing support into the community, working with the Food Banks and Love Driffield to offer advice and blood pressure checks in a familiar, welcoming setting.

By being out in the community, the team can connect with people who may not otherwise seek out health advice, offering an opportunity to ask questions, check their blood pressure and find out more about the support available to them.
More than a blood pressure reading

The sessions are about more than BP checks. They provide an opportunity to talk, listen and understand what matters to people, whether that's a health concern, help accessing local services or support with wider issues affecting their wellbeing.

Our Social Prescribers can help people connect with community activities, local organisations and practical support, while the PCN Care Coordinator can help people navigate services and access the support they need.

Working alongside organisations such as Love Driffield and the other Food Banks helps us reach people in places they already know and trust, making these conversations feel more accessible and less formal.
Working together locally

This is a great example of neighbourhood partnership working in practice. By bringing together primary care and community organisations, we're creating opportunities to support people earlier, improve awareness of health and wellbeing, and help people access the right support for their individual circumstances.

It's about taking a proactive approach, building connections within our communities and recognising that good health is influenced by much more than access to a GP appointment.
