Welcome to the website of the Yorkshire Coast and Wolds Primary Care Network (PCN).
Our four local practices are working together to transform health services in the Yorkshire coast and wolds area. Whilst aiming to improve all aspects of health provison, we have identified some clear local priorities that will receive our special attention.
PCNs were formed to provide support to groups of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in a local area. We build on existing primary care services to enable greater provision of proactive personalised, coordinated and more integrated health and social care for people living close to home.
We are working hard to transform health services in our local area. We are guided by national imperatives which are informed by health data - but focused on how such insights impact in our localities. Importantly, our practices understand what is important for their patients from direct contacts on a daily basis.
Our initial priorities are focused in mental health in rural communities, early cancer diagnosis, enhanced access to primary care services, health inequalities, smoking cessation, promotion of healthy living and prevention of ill-health.
The success of a Primary Care Network is not just a matter of improving teamwork between health professionals to improve services. It is also vitally dependent upon the engagement of patients in adapting to healthy lifestyles to help prevent ill-health.
Also, by raising awareness and encouraging patients to access screening services, we can improve lives through early diagnosis.

When you feel unwell or have sustained an injury, it is often difficult to know which is the right service to approach for help. Making the right choice is important to getting the most appropriate care at the right time. In choosing the right service, you will also be responsibly helping the NHS to manage demand efficiently. If you can, use on-line services at first.
